URGENT QUESTIONNAIRE

In the Staff Memo #3 that was emailed on the 20/12/21 it was clearly stated that replies needed to be sent via SMS ONLY. 
So to all those staff that have replied to this staff memo email address, you will need to SMS your reply as this email address is not monitored by management.

1. Are you willing to work with a client who has tested positive to COVID?

2. If you work with a COVID positive client will you isolate at home (like hospital personnel) or will you require alternative accommodation?
